We value your business and thank you for your confidence in choosing us as a supplier of physiotherapy services.  As a client/patient, you trust us with your personal information.  We respect that trust and want you to be aware of our commitment to protect the information you share in the course of doing business with us.

 

How we collect, use and disclose your information

 

When you agree to treatment from us, you share personal information so that we may provide you with products and services that best meet your health needs.  We assume your consent  for us to use the information in an appropriate manner.  We may use and disclose this information in order to:

 

               Communicate with you

               Communicate with your Doctor

               Communicate with your Specialist

               Communicate with your Insurance Company (as permitted)

               Act as required or authorized by law

 

What we will NOT do with your information

 

We do not sell customer information to anyone.  Nor do we share client information with organizations outside of this company.


We strive to protect your personal information

 

All employees who are granted access to customer records understand the need to keep this information protected and confidential.  They know they are to use the information only for the purpose intended. This expectation is clearly communicated.


We've also established physical system safeguards, along with the proper processes to protect customer information from unauthorized access or use.

 

Your Privacy Choices

 

You may withdraw your implied consent at any time (subject to legal or contractual obligation and on providing us reasonable notice) by contracting the Privacy Officer indicated below. Please be aware that withdrawing your consent may prevent us from providing you with the requested product or service.

 

We may occasionally use your personal information to tell you about other services that we believe may be of interest to you. If you would rather not receive this type of communication, please let us know through our Privacy Officer, Rob Landers.

 

Confidentiality 


The Physiotherapy Staff is to follow the Confidentiality Policy.

 

Information of a confidential nature concerning a patient, staff or any other clinical business is to be held in strict confidence, and is not to be discussed with anyone other than those persons authorized to review such information in the course of their duties.

 

Included within this policy is the expectation that staff restrain from e-mailing information containing patient identification.
